Recall on Ohlin's forks because top cap can work itself loose, causing air shaft to come out and injure the rider.

Reported that this has happened 4 times already. 4!!!! That 4 too many for something people have been designing since 1999.

Sounds like they failed to torque the caps to proper amidst by.

It happens. GM just recalled 1 million trucks. Japanese airbags turn into claymores. An improperly torqued cap seems within the realm of possible.

 

And sadly enough a lot of this probably happens because the manufacturers want to save a penny per vehicle.
